If you are a Member of the Club you will also need your Membership Number.

Telephone Number, Daytime and Evening

Date and Time room required

Number of People attending 

If Catering Facilities will be required

 

The Form will be reviewed by the Secretary who will contact you to discuss if the room is available / suitable or if alternative arrangement can be made etc.
